Don't have any answers, but maybe-
Contact EFI directly and ask Doug questions?
The EFI PMS box isn't the right one for your car? I don't know if there is a difference in boxes between VTEC/non-VTEC engines for example. Maybe you have the wrong one? Do you have anchor points at 2200, 4000, 6000 and 7800? It would seem odd to have an anchor point above redline?
Does the PMS unit register an error code? Error codes erase after you turn the power off so you'd have to run the car in your problem range and remember to check before turning off the car.
I still don't know how you can be raising the rev limit without running in standalone mode. If you're not running standalone mode, it would seem that the unit wouldn't work as it is simply altering the fuel/timing values of the stock base map - I'd guess there wouldn't be any RPM/load values in the stock ECU past redline to be modified by the pMS unit?
What ECU are you running?
Can you at least take a friend in the car and watch the PMS values on the handheld unit? The readings are very transient admittedly, but maybe something will show like your timing going to zero, injector pulse width falling off, etc?
Can you return the ignition back to stock to take that out of the problem equation?
Try a dyno shop that has data acquisition that can be viewed/displayed/replayed? Get Interacq and datalog it yourself which I would guess you could do on the street and then send it to whoever did your tuning?

Defcon: Nice work. You're exactly where I was at with stock injectors...
Last time I dyno'ed I was at 186/153... Hoping in the 190 range now (lighter flywheel, hi-flow cat) but I'm still running 235cc injectors... thinking about getting larger ones but I still have the off throttle hiccup (because I'm using the JR Map Mod). Not sure what I'm gonna do. I think with the B18B1 at some point we are limited by flow problems (our heads), which seems to be around 190 HP (stock block of course).
Where is your redline??? When I dyno'ed, my car's fuel cut was at 6850... your's appears to be at 7250???

defconsc-
I had no problems like you at the dyno today with my PMS unit. I must say I am very happy with my purchase of the PMS system. I can see where Interacq would be helpful to really dial things with more granularity and having the datalogging, but I think the handheld controller is adequate and incredibly user friendly and effective, especially if the dyno can do datalogging separately. I pretty much created a flat 12:1 fuel curve and my torque is flat all the way out to redline. Gained some power up top too. I'm going to go back and tune a separate map for race fuel using the base map for crappy 91 octane pump gas I did today in about an hour and a half.
Have you been able to resolve anything yet?
